First stage underperformed due to an engine leaking fuel and running at low thrust. Corrected by onboard guidance extending the duration of first stage flight, however this was not communicated to the upper stage, which attempted to separate at the normal time, whilst the first stage was still firing. Recontact prevented the upper stage orienting for its burn, leading to payload being deployed into useless orbit.[2]
